Proof of U.S. citizenship or lawful status and identity, for example a certified state birth certificate or a valid U.S. passport. Originals only: no faxes or photocopies. Two documents from two different sources with your Nebraska address (no P.O. Box). Under 21, your parent’s or guardian’s address documents are accepted. Your Social Security number (for example your Social Security card, a W-2 or an SSA-1099).
